I, ___________, with ID ___________, hereby authorize my child or ward ___________, with ID ___________, to drive or to take part in the activity as a passenger with an adult or an already-authorized minor over 16, on the jet ski at Jet Ski Tours Salou & Cambrils.
I acknowledge that: The legal guardian submitting this authorization confirms that the minor who drives is over 16 years of age and assumes full responsibility for them, their actions, and their safety.
Furthermore, I understand that my child or ward will be responsible for using or operating the jet ski safely and complying with all established safety, navigation, and conduct rules. I also accept responsibility for any physical or financial harm my child may cause to third parties during the activity.
To verify the authenticity of this authorization, the company also requires a photo of the legal guardian's ID or Passport. It will be permanently deleted from our database after the excursion.

Any false information provided in this form, including a falsified signature or a false declaration of parentage or guardianship, may be reported to the competent authorities and prosecuted accordingly.

By signing and submitting this form, the legal guardian assumes full and exclusive responsibility for the minor's participation in the activity. Jet Ski Tours Salou & Cambrils is totally and absolutely exempt from any liability for any accident, injury, property damage or harm of any kind resulting from the minor's participation, including damage caused to third parties. The company's liability is expressly waived in its entirety by the signing of this authorization.
